Your timing may be off. Make sure you have him eating and then out right away, also how often are you feeding? Water should be taken away after 7 or 8 PM you may want to use your crate more, too, if you take your eyes off him, he needs to be crated. like during dinner, or if you are cleaning, etc.
Note when he tends to have the accidents and try and modify his out times to that.
pups usually can hold urine 1 hour for every month, so, he should be holding at least 6 hours, but this is only and average.
We are having the same problem with a Dachshund we are showing, she is 9 months old and all of a sudden she started going off schedule, we think it is that, we put her in a larger crate, and she just has the option, like going in another room if they are out free, while you are NOT watching :0)
We will try a smaller crate for our problem child.
